Intended Use
For the quantitative in vitro determination of total iron binding capacity (TIBC) in serum. This product is suitable for use on the RX Series instruments which includes the Rx Daytona and Rx Imola.
Clinical Significance
Measurements obtained by this device are used in the diagnosis and treatment of anaemia.
Total Iron Binding Capacity (TIBC) is tested alongside serum iron to determine the bloods capacity to bind iron with transferrin. It is therefore an indirect measure of transferrin particularly important when testing for anemia and iron deficiency.
In individuals deficient in iron TIBC is increased however in iron overload the TIBC will be low or normal.
High levels of transferrin or a high TIBC is also associated with pregnancy and the use of the contraceptive pill. A low level of transferrin in the blood or a low TIBC may be contributed to hemochromatosis, anemia, poor nutritional status, liver disease or kidney disease.
Principle
Randox TIBC assay uses two reagents in a sequential process that may be monitored spectrophotometrically.
Step 1:
The first reagent (R1) an acidic buffer containing an iron-binding dye and ferric chloride is added to the serum sample. The low pH of R1 releases iron from transferrin. The iron then forms a coloured complex with the dye. The coloured complex at the end of this first step represents both the serum iron and excess iron already present in R1.
Step 2:
Reagent 2 (R2), a neutral buffer is then added, shifting the pH and resulting in a large increase in affinity of transferrin for iron. The serum transferrin rapidly binds the iron by abstracting it from the dye-iron complex. The observed decrease in absorbance of the coloured dye-iron complex is directly proportional to the total iron-binding capacity of the serum sample.
