Intended Use

For the quantitative in vitro determination of ω-amylase in serum, plasma and urine. This product is suitable for use on the Hitachi 917 and Hitachi Modular P.


Clinical Significance

The amylases are a group of hydrolases that split complex carbohydrates constituted of ω-D-glucose units. The two recognized types of amylases are ß-amylase (e.g. plant and bacterial exoamylase) and human ω-amylases which can attack the ω-1, 4-linkage anywhere along a polyglucan chain. Human ω-amylase consists of two major isoenzymes, pancreatic and salivary, which are encoded by two different genes. Pancreatic amylase is synthesized only in pancreatic tissue by acinar cells, however the salivary amylase is found in numerous locations, such as salivary glands and female genital, pulmonary and malignant tissues.

Amylase measurements are used primarily for the diagnosis and treatment of pancreatitis (severe inflammation of the pancreas) as well as pancreatic diseases. Amylase levels rise quickly at the start of a pancreatitis attack and fall after around 2 days. Elevated levels may also be associated with cancer of the pancreas, a perforated ulcer, gallbladder disease, mumps or ectopic pregnancy.


Principle


The method uses ethylidene blocked p-nitrophenyl-maltoheptaoside as substrate. The indicator enzyme α-glucosidase, used to release the p-nitrophenol, is also employed in the method. The terminal glucose of the substrate is chemically blocked preventing cleavage by the indicator enzymes.
